Gingival smile is an aesthetic concern that affects the beauty, confidence, and hygiene of the patient due to excessive gum exposure while smiling. Surgical techniques such as gingivectomy and osseous resective surgery are employed for its treatment. Recent technological advances, such as surgical guides and digital smile design, enhance clinical crown lengthening procedures. These digital customized guides allow for more precise planning, reduce soft tissue recessions, and enhance aesthetics and function; also makes easier communication with the patient and establish realistic expectations. A clinical case of a 26-year-old female patient with gingival smile is reported, where clinical crown lengthening surgery was performed using a surgical stent, and the results matched the planned outcome without any intraoperative or postoperative complications. The use of digital customized guides has proven to be safe and effective, enabling more precise and personalized interventions. These advancements represent progress in aesthetic dentistry and are expected to play a significant role in periodontal surgical procedures.
