Management of an oroantral fistula: clinical case report

Introduction: An oroantral fistula is an abnormal pathological communication between the oral cavity and the maxillary sinus. It may occur as a complication of tooth extraction, as well as a result of deficient primary healing, dental infections, radiotherapy, osteomyelitis, trauma, or iatrogenic complications. The presence of a fistula results in a permanently open maxillary sinus, allowing the microbial flora to pass from the oral cavity into the maxillary sinus, which then becomes inflamed with all possible consequences. Objective: To present the appropriate technique for the surgical management of an oroantral fistula located in the upper right molar region of a 27-year-old female patient. Methodology: This research is a clinical case report framed within the field of clinical and corrective dentistry, supported by a longitudinal quasi-experimental design. The vestibular advancement technique with a Bichat’s fat pad graft was applied, chosen among the indicated techniques for closing an oroantral fistula. Results: The patient did not present recurrence of the communication, showing good healing and tissue appearance, demonstrating the effectiveness of the vestibular advancement technique and Bichat’s fat pad graft for the treatment of medium-sized oroantral fistulas.Conclusions: It is concluded that this technique is viable, safe, and easy to manage for the resolution of cases of oroantral communication.
